Steps
  1. Activate Ashnod's Altar by sacrificing another nontoken Hydra that does not have Nim Deathmantle attached, adding {C}{C}.
  2. When the Hydra dies, Polukranos and Nim Deathmantle trigger.
  3. Resolve the Polukranos trigger, creating two 3/3 Phyrexian Hydra creature tokens.
  4. Activate Ashnod's Altar by sacrificing a Phyrexian Hydra, adding {C}{C}.
  5. Resolve the Nim Deathmantle trigger from step 2, causing you to pay {4} to return the nontoken Hydra from your graveyard to the battlefield, and attach Nim Deathmantle to it.
