Steps

  2. Scurry Oak's second ability and Moss-Pit Skeleton trigger.
  3. Resolve the Scurry Oak trigger, creating a 1/1 Squirrel creature token.
  4. Resolve the Moss-Pit Skeleton trigger, putting it from your graveyard on top of your library.
  5. Activate Altar of Dementia by sacrificing the Squirrel token, causing you to mill Moss-Pit Skeleton from the top of your library.
